MONTHLY MEMBERSHIP FEE SCHEDULE
- Children Age 0-18
- $85/month
- Adults 19 and older
- $135/month
- Family Maximum
(2 adults + 2 children) - $325/month
- Home visits when indicated/feasible for additional charge of $50 per visit.
- Membership fees will be due on the 1st of the month and will be prorated for those joining after the 15th of the month.
- A one-time enrollment fee equal to one month’s membership will be charged at the time of contract signing.
- Minimum enrollment: 3 months.
- If you are an existing patient of Dr. Garofalo’s, you will still need to sign a contract to establish at Hydrangea Family Medicine, and have your records transferred to the new office.

Services
Everything below is included in your monthly fee. There are no hidden fees or copays!
Routine/Preventive Care
Dr. Garofalo will conduct your annual wellness exams, school/camp physicals, cancer screenings, and more.
Acute/Urgent Care
You can rest easy knowing you’ll have same or next day access for sick visits so you can avoid the ER or urgent care center.
In-Office Testing
Dr. Garofalo will be able to perform strep tests, flu/covid testing, urinalysis, pregnancy tests, EKG and more.
Chronic Disease Management
Dr. Garofalo is skilled in managing chronic diseases like diabetes, hypertension, depression/anxiety. She also has a special interest in Ehlers-Danlos and its related conditions.
Pediatrics
As a family physician, Dr. Garofalo loves caring for kids of all ages for both well visits and acute issues, including vaccinations.
In-Office Procedures
Skin tag removal, wound care, incision/drainage, ear wax lavage, wart removal, etc. are all included in your membership.
Easy and Convenient Access
In-person and virtual visits, direct texting, calls and portal messages will keep you in touch with the physician you know and trust, with less wait time and longer appointments to address your concerns.
